Job Summary
\nThe Home and Community Bases Services Case Coordinator is responsible for overseeing the services provided to a caseload of clients daily. This responsibility includes interfacing with clients/families/employees to ensure quality service delivery. Coordinates and ensures the scheduling of client cases on a daily basis. This includes selecting the appropriate caregiver, communicating with the family or client on an ongoing basis regarding staffing.
