Summary Of Position:
Serves as an integral member of the multi-disciplinary team and a core facet in the continuum of care for Behavior & Primary Healthcare Coordination (BPHC)-enrolled clients. Provides individual, supportive coaching to improve physical health status; coordinates client care with team of internal/external providers; aids clients in accessing/utilizing resources; and promotes the best outcomes for our clients.

Years of Experience: Two years of direct service experience working or volunteering in a human service organization.
